" Prevention and Management of Common Implant Complications "

Implant complications have become a daily challenge for many practicing dentists. This presentation is designed to provide most updated information on how to prevent and manage of this common emerging problem. Steps utilized to avoid these common implant complications will be illustrated. The newly developed decision tree on how to select and manage the implant bone loss and peri-implantitis will be demonstrated. Techniques such as adjunctive use of chemotherapeutic agents, apically positioned flap with or without implantoplasty, implant surface detoxification (e.g., lasers), guided bone regeneration, soft tissue grafts, implant removal as well as re-implantation will be presented.

Dr. Christian Coachman
Dr. Christian Coachman graduated in Dental Technology in 1995 and in Dentistry at the University of São Paulo/Brazil in 2002. He is a member of the Brazilian and American Academy of Esthetic Dentistry. Moreover, Dr. Coachman attended the Ceramic Specialization Program at the Ceramoart Training Center, where he also became an instructor.
In 2004, Dr. Coachman was invited by Dr. Goldstein, Garber, and Salama, of Team Atlanta, to become Head Ceramist of their laboratory, a position he held for over 4 years.
Dr. Coachman worked with many leading dentists around the world as Dr. Van Dooren (Belgium), Gurel (Turkey), Fradeani (Italy), Bichacho (Israel), Ricci (Italy) and Calamita (Brazil).
He is the developer of the Digital Smile Design, the Pink Hybrid Implant Restoration, the Virtual Planning Center, Emotional Dentistry, Interdisciplinary Platform and Smile Donator concepts, conceptual developer of DSD App and founder of the DSD-Digital Smile Design company.
Currently divides his time between his family Dental Clinic in Sao Paulo-Brazil and the Digital Smile Design headquarters in Madrid-Spain. He is the Director of the DSD Residency program with continuing education courses for dental professionals from all over the world. Also works as a consultant for Dental companies and offices, developing products, implementing concepts and marketing strategies.

Dr. Michael Sonick
Michael Sonick, DMD, is an internationally known, highly regarded authority in the field of dental implantology and periodontology. He completed his undergraduate degree at Colgate University. He received his DMD at University of Connecticut School of Medicine and his certificate in periodontology at Emory University. He received his implant training at Harvard University as well as the Branemark Clinic in Gothenburg, Sweden.
A full-time practicing periodontist in Fairfield, Connecticut, he is also a frequent guest lecturer in the international program at New York University School of Dentistry and the University of Connecticut School of Dental Medicine. He lectures worldwide on the subject of aesthetic implants, periodontal plastic surgery and guided bone regeneration. He is the co-editor of the multi-language textbook, Implant Site Development. He serves on the editorial boards of numerous journals including the Compendium of Continuing Education and the Journal of Cosmetic Dentistry, and also writes and publishes numerous papers in peer-reviewed journals. He is a diplomate of the International Congress of Oral Implantology.

The maxillary anterior dental implant is perhaps the most challenging for the clinician. Knowledge of esthetics, bone grafting, gingival grafting and precise implant placement is essential to achieve an implant restoration indistinguishable from the natural dentition. In modern implant dentistry, mere osseointegration is not enough. We tolerate no less than perfection. Thus the position that creates the most beautiful and functional prosthesis dictates the location of the implant, not the ridge morphology. Today, we simply grow bone where we desire it.
Techniques that are now available to augment hard and soft tissue at the various phases of treatment will be shown. Techniques will include particulate bone grafting, guided bone regeneration with bio-absorbable and non-resorbable membranes and the use of the osteotomes and ridge expanders. Soft tissue procedures to assure optimal esthetic reconstruction before implant placement, at implant placement, at second stage surgery and after second stage surgery will also be demonstrated. These include flap designs to optimize ideal soft tissue profiles, three dimensional connective tissue grafting, and the use of number of second stage surgical flap designs. The requirements and limitation of papilla regeneration between implants, implants and natural teeth, and implants and pontics will be shown. -
Dr. Paul Fletcher
Dr. Fletcher is a periodontist who practices in New York City and as a member of Specialized Dentistry of New York, a group implant, cosmetic specialty practice. He is an Associate Clinical Professor at the Columbia University College of Dental Medicine, where he teaches periodontics and implant dentistry on a post-graduate level. Dr. Fletcher has had more than two dozen papers published in the dental literature and has lectured on periodontal prosthetics and implant dentistry both nationally and internationally.

As the number of implants that have been placed worldwide increases and as the number of years implants have been in place increases, the incidence and prevalence of inflammatory peri-implant disease has been rising. Whether you are placing or restoring implants it’s imperative you have an understanding of the factors involved in the initiation of peri-implant disease, are able to diagnose the problem in its incipiency and can then take the proper steps to arrest its progression.
At the end of this presentation the participant will:
1. have an understanding of the etiology, microbiology and histopathology of peri-implant disease, and its progression from mucositis to peri-implantitis.
2. understand which instruments and chemotherapeutic agents are best suited to debride, detoxify and decontaminate an affected implant.
3. understand the indications for non-surgical and surgical therapy, and understand the specific implant surgical techniques utilized to optimize the result of treatment.
4. know how to establish an office protocol for the early recognition and diagnosis of peri-implant disease as well institute Cumulative Interceptive Supportive Therapy (CIST) to stop mucositis before it progresses to peri-implantitis.

Present-day knowledge and techniques used to restore function and aesthetics following the loss of teeth is vast and extensive. Over time, the aesthetic outcome has grown into one of the most challenging tasks for clinicians. The knowledge of aesthetics, soft and hard tissue augmentative techniques, precise implant placement, and proper implant site preparation are critical elements to a harmoniously predictable outcome in the long-run.
(1)Endosseous implants cannot be successful with inadequate bone support, and much effort goes into maintaining and regenerating even small amounts of alveolar bone. Imparting the knowledge that successful endodontic therapy can not only prevent bone loss, but also regenerate bone by eliminating inflammation and disease, we will discuss the use of nonsurgical and surgical endodontic therapies as adjunctive therapies to implant placement. Evidence from the endodontic literature and cases will be utilized to present a novel approach to multidisciplinary care.
(2)An interdisciplinary approach which combines periodontal-restorative and ortho-restorative (aligner orthodontic treatment) interactions with strategic treatment planning have become the main focus in the three-dimension implant dentistry.

Dr. Jarshen Lin is currently the Director of Extramural Education and the Director of Pre-doctoral Endodontics at the Harvard School of Dental Medicine at Harvard University. He is also the Chief Endodontist for Massachusetts General Hospital (MGH) of Harvard Medical School.
He has served as a member and a chair of the committees of the U.S. National Board Dental Examination Part II, the American Dental Education Association, the American Association of Endodontists, and the Academy of General Dentistry.
Beyond his teaching and clinical activities, Dr. Lin serves as an editor for various dental textbooks and is a member of the editorial boards of several journals. His most recent textbook publications include Endodontics Review by Quintessence Publishing in 2016 (the 2nd edition will be published in early 2020), and ENDODONCIA. Ciencia y tecnologia.
During his tenure at Harvard, Dr. Lin received fifteen local, national and international teaching awards, including the Edward Osetek Educator Award by the American Association of Endodontists. This award is the highest honor given to young educators in the field of Endodontics. He has been selected as a “Top Dentist” by Boston Magazine every year since the election by Boston magazine.

Dr. Yvan Poitras
Dr. Yvan Poitras, DMD, graduated from Université Laval, Canada in 1984. He is the founder and president of the Canadian Implant Institute, a continuing education center exclusive to oral implantology for 27 years. Very engaged in this field, he shares his time between his exclusive private implant practice, teaching, international conferences, writing numerous papers, articles, as well as research & development with patented innovations to his name such as the Poitras Surgical Template, the Poitras Stitch, the Swedging Technique and the CAD-CAM Subperiosteal ImplantTM, subject of his APAID lecture. Dr. Poitras is attached to the research group in biomechanics/biomaterials at École Polytechnique, Université de Montréal, co-director of the Franco-Canadian implantology university degree at Université d’Évry-Val-d’Essonne, France. To treat the very common severe vertical atrophy of the posterior crest of the mandible, where the challenges of bone grafting render a slim prognosis and the insertion of endosseous implants is proscribed, Dr. Poitras and his team developed a unique solution which requires only one surgery, the CAD-CAM Subperiosteal ImplantTM. This bone onlay implant is fully designed and manufactured with state-of-the-art CAD-CAM technologies. From a CBCT-Scan, superposed with a digital impression, it is computer designed with the purpose of the best prosthetic result and then manufactured in a block of Ti 6Al 4V. It perfectly matches the morphology of the patient's bone. The integrated transmucosal abutments make it easy to restore. The macro design of the implant favors osseointegration as well as guided bone regeneration which overlay its structure with new bone to become inlay. " Innovations in the Implant Design "

During the last decades, the implant options have provided various tools in the prosthodontic treatment plan. The implant gives more stable support than the fixed partial denture as well as the removable denture. The implant saves adjacent teeth as intact, and supports the abutments of the bridge. The implant delivers more vertical stops than the removable denture, making the whole occlusion more stable. The implant never mesially shifts unlike natural teeth.
The implant prosthesis also has some problems. Screw loosening or fracture, veneered porcelain fracture, and prosthesis fracture are somewhat related to its inherent mechanical handicap, which is relatively weak buffering capacity against occlusal force compared to teeth that had periodontal ligaments.
The innovations in the implant design have proceeded in terms of overcoming its weaknesses. Screw connection between two components, microgap leakage, and weak periimplant defence against bacterial infection made the implant prosthesis susceptible to biologic and mechanical complications. Shifting the microgap away from the periimplant tissue, one-piece abutment-implant, or zirconia implant and implant prosthesis were those efforts of improving the longevity of implant.
What will be the ideal implant design in the near future ? Retrievability, biocompatibility, or mechanical stability - which one is the most important characteristics of the implant prosthesis ? I hope you come and discuss with novel audiences during my presentation. -

" Management of Surgically Compromised Condition for Dental Implant "

Surgically compromised implant cases always shared more risks than the usual cases. Care should be taken in the design of
treatment modality and follow-up. The clinical procedure is based on a scientific biologic basis and especially the biologic limits. The disuse atrophy in the edentulous ridge predisposed a gradual loss of the bone level and the keratinized gingiva that challenged the dental implant rehabilitation. Both soft and hard tissue management may be needed before implant rehabilitation was possible. The cases suffering from traumatic injury, mostly are from motor bicycle accidents, also presented a similar focal oral environment as the atrophic ridge that may need both vestibuloplasty or bone augmentation. Difficulties may be encountered in these cases due to the need for tissue management in such a limited alveolar area with existing adjacent teeth. In this presentation, several surgical indications and techniques will be introduced. A logic sequence of reconstruction is composed of autogenous or allogeneic bone graft, soft tissue management and implant surgery, and several special procedures to achieve a satisfactory outcome of the treatment. -
Dr. Shohei KasugaiProf. Shohei KASUGAI graduated from Faculty of Dentistry, Tokyo Medical and Dental University (TMDU) in 1979. Then, he was studying as a postgraduate student and working as an assistant and associate professor in Department of Pharmacology. He was temporarily working as a post-doctoral fellow in MRC Group in Periodontal Physiology, University of Toronto from 1989 to 1991. In 2000 he was promoted as a professor for the science of implant dentistry because of his strong background in bone tissue. He is focusing his study on dental implant materials and tissue regeneration related to dental implant treatment, especially bone regeneration. He published more than 170 scientific papers in peer-reviewed international journals. He was awarded in International Dental Association (1990), in Japanese Association of Oral Biology (1994), and in Academy of Osseointegration (2009, 2010 and 2011). He lectured all over the world in international meetings and well-known universities including Harvard University and University of Gothenburg. In addition to his extensive research and educational rolls in TMDU, he is treating many patients with dental implants.
" Seven Keys for Long Term Good Prognosis in Dental Implant Treatments "

Currently, oral rehabilitation with dental implants for partially or fully edentulous patients is very effective. Although survival rate of implants is high, mechanical and/or biological complications are quite frequent. Especially, chipping of prostheses and loosening of screws occur sometimes. Furthermore, peri-implantitis, inflammatory destruction of the surrounding tissues of dental implants is a great concern. Observing problem cases of dental implant treatments, I realized that the following 7 keys are required for long term good prognosis.
1. Proper examination and treatment planning
2. Proper implant placement
3. Presence of keratinized mucosa
4. Proper loading
5. Cleanable prosthesis
6. Good oral hygiene and controlling periodontitis
7. Proper maintenance

Soft tissue deficiency is the main factor causing poor esthetics around implant. It is primarily a volumetric inadequacy which consists of concavity and recession of labial tissue and/or loss of interdental papilla. It could occur in 3 phases namely, prior to the 2nd stage, during the provisional phase and after permanent restoration. The surgical management in each phase can be very different. Poor implant positioning and presence of infection complicated the management.
It is the purpose of my presentation to provide surgical solutions to correct the soft tissue deficiencies with the following 3 variables, timing (surgical exposure, provisional and implant complications), type of tissue loss (midfacial and/or interdental) and status of tissue around implant (healthy or diseased). Majority of the presentation will be focusing on poor esthetics on implant complications. Specific surgical consideration and techniques will also be presented for implant placed too far buccally. The outline of my presentation will be laid out in the following 5 chapters:
I. Introduction: indications, diagnosis and treatment planning
II. Surgical procedures utilized for 2nd stage exposure: APF, semilunar, Rolled technique, VIP-CT, CTG via crestal pouch
III. Surgical procedures utilized during provisional phase: Rolled technique, CTG via lateral pouch or apical pouch
IV. Surgical procedures utilized for implant complication: CTG via lateral pouch or apical pouch, VIP-CT, decontamination techniques, GBR
V. Concluding remarks: graft survival principles, evidence based but not limited
Soft tissue deficiency always comprises of the loss of both soft and hard tissue. However, for predictability, the surgical solution may direct toward correction of the extrinsic gingival characteristics in particular size and contour utilizing soft tissue graft only. When there is evidence of disease status with non-resolved probing depth should one consider using both soft and hard tissue augmentation techniques to regenerate lost soft and hard tissues.
In summary, the degree of difficulties increases as the later appearance of poor esthetics, the demand for papillary improvement and the presence of contaminated implant occurs. As all we know, location, depth and angle of dental implant is so important.
Because most of time, it is impossible to change and also affects final result, long term success, and maintenance.
Recently, many clinicians are using CT - based static surgical guide not only for implant placement but also for simulation before implant placement.
Static guide still have some errors and limitations but better than ‘free - hand placement’ and also can be used for flapless approach and immediate loading in limited cases.
The digital impression device has become more accurate and popular, so digital impression of implants is widely has been used.
For digital impression of implants, there are many different ways to do. We can use scan bodies instead of impression coping or to scan prefabricated abutment directly in the oral cavity that has already registered in CAD / CAM software library in simple case.
The impression using coded healing abutment has also been introduced for a long time ago.
The advantage of this method is that it is possible to make prosthesis during the period of waiting for osseointegration because the timing of impression scan of coded healing abutment is more free than other methods.

Implant dentistry has been developed for decades. With the improvement of surface treatment and advancement of guided bone regeneration, surgical considerations of implant placement has evolved from bone-driven to prosthetic-driven.When treating patients with failing dentition, precision of implant placement is always challenging due to lack of solid intra-oral anatomical landmarks. Stereolithographic guide has been widely used in full mouth implant rehabilitation for better accuracy of implant placement. However, difficulty is frequent encountered such as stabilization of the guide, limited range of mouth opening and cooling system of the drills could potentially jeopardize its utilization. Dynamic navigation system serve as another treatment modality in providing precision dentistry in implant surgical phase. Integration of treatment plan utilizing digital tools and clinical application of navigation system will be discussed in this presentation. -

The aim of implant prosthesis is to replace teeth that loss to maintain patients’oral functions with their previous occlusion. However, some in particular patients who have dentofacial deformities and loss multiple teeth or severe alteration of the edentulous ridges, conventional prosthesis to restore their previous occlusion may be not favorable.
Oral rehabilitation with implants can be done not just to replace the loss of dentition but also can change the patients to a more favorable occlusion and facial profile without the need for orthodontic treatment. Some adjunctive maxillofacial surgical procedures may be needed for a favorable outcome. However, the treatment should be well planned. Patient’s desire, cooperation and compliance to achieve a predictable and satisfactory result are most important before making treatment plan.
This presentation will demonstrate the implant treatment in some patients with dentofacial deformity and multiple tooth loss, patient selection, treatment plan, surgical treatment and the outcome. It may be another treatment option for this particular group of patients to be considered. -

The two-stage protocol created by Dr. Branmark in 1980’s are still the guidelines followed by most dentists. In recent years, large quantities of studies and clinical evidence of immediate loading confirmed that the results of two-stage and one-stage protocol for dental implantology are the same. However, one-stage protocol is closer to patient’s needs, which are to reduce surgical operation, restore appearance and masticatory functions rapidly. By the way, with one-stage protocol, practitioners can relatively reduce time consumption and the costs of materials.
The lecturer has over thirty years of long-term and massive one-piece dental implantology experience. He will share the clinical application of his Tony Simplified Concept Technique (TSCT) in One-operation Implantology (All-in-one) at full arch reconstruction with participants. This method does not need complicated computer-aided digital assembly to manufacture surgical and prosthetic device.
As to the surgical part, this method is based on Tony’s one-operation implantology (All-in-one) approach protocol which include the concept of bone-driven implant site, one-stage immediate loading, one-abutment one-time (final abutment place at the time of surgery) and combine GBR procedure in one operation, which focuses on achieving the least surgical operations and fulfilling the patient’s desires (reduce suffering, restoring appearance and masticatory functions rapidly) .
As to the prosthetic part, TSCT method is based on simple and traditional dental template techniques, almost all general laboratory technician can perform these works easily. In recent days also, we use digital scanning and digital sculpture to complete provisional and final prosthesis. According to hundreds of clinical cases from the past decade, the success rate of TSCT full arch reconstruction method is up to 97%. This simplified full arch dental implant rehabilitation is applicable to general dental clinics because it does not require high technology, high complexity, and pricey capital expenditure. -

Functional brain MRI disclose a twilight of Pandora’s box of TMDs. Toward the era of artificial intelligence (AI) and augmented reality (AR), TMJ surgeons have lots of visions to be accomplished. Sinus augmentation using variable bone substitutes has been considered as predictable method to reconstruct the pneumatized sinus cavity for last many years.
However, bone graft - as foreign material – can cause severe complications such as postoperative infection, poor bone regeneration and failure of bone regeneration in the augmented sinus. Postoperative infection sometimes requires long time and patient will be in great pain until it is resolved
Recently many clinical and animal studied proved new bone formation in the sinus without bone graft and 0 % of postoperative infection. Sinus augmentation without bone graft has numerous advantages : 1) 100% vial bone formation is gained, 2) risk of cross contamination from animal or human origin bone is eliminated 3) postoperative infection is not developed, and 4) fast new bone induction is achieved.
This presentation will demonstrate new bone formation in the sinus without bone graft using lateral window technique and crestal approach - hydrodynamic piezoelectric internal sinus elevation ( HPISE ) - with clinical, radiographic, and histologic evidence from animal and human. -

Placing dental implant in aesthetic zone is a bit technical sensitive if the surgeon aims for the best outcome. The use of 3D CBCT scanning incorporating with guided surgery has been proven to be successful for implant surgery in aesthetic cases. However, the use of conventional surgical guide is still widely used where the modern 3D scanning is not available. Clinically for implant placement in aesthetic zone, one could meet an unlucky day due to some reasons such as: lack of experience of surgeon, poor design of conventional surgical guide, or narrow bone space that limit implant placement into an ideal position.
Those error implants of wrong located position, improper angulation or incorrect depth are the most common problems that many clinician always face. Preventive measure could be prepared before disastrous outcome of compromise aesthetic result. Good diagnosis and treatment planning, understanding aesthetic dentistry, improving surgical skill and experience of implant surgeon and finally improving restorative ability of prosthodontist are keys to success for implant in aesthetic zone.
However, when error implant happens, possible management and solution could be activated to solve the problem. A good result after error implant will depend on proper management and prosthodontic skill of clinician who can turn any problematic implant into a satisfied result. -

" Laser Application in Periodontics and Peri - Implant Therapy "

Laser applications for periodontal and implant therapy have gradually expanded as a result of the increase in published basic and clinical. Laser can be used adjunctively for initial periodontal therapy, not only to debride connective tissue and epithelium within periodontal pockets, but also to inactivate bacteria that invade the periodontal tissues. In addition, erbium lasers can ablate calculus with efficiency comparable with that of hand or ultrasonic instruments. Esthetic gingival procedures, such as recontouring or reshaping of gingiva in peri-implant soft tissue, peri-implantitis, and second-stage implant surgery, can be performed by lasers. For esthetic gingival procedures, an erbium laser can be more safely utilized because of its minimal thermal side effects than CO2, diode and Nd:YAG lasers. If small and delicate contact tips are used, the amount of soft tissue ablated with an Er:YAG laser can be controlled with more precision than with the other lasers; in addition, with less thermal alteration of the treated surface, wound healing can be more rapid. Furthermore, when used in combination with a surgical microscope, an Er:YAG laser with low energy can almost completely eliminate even small areas of gingival troughing , by carefully irradiating the delicate and fragile gingival margin and papilla without causing postsurgical gingival recession. -

Rehabilitation of the edentulous ridge with implant - supported fixed complete dental prostheses represents a scientifically and clinically validated treatment. However, due to the limitation of the anatomical structures in the extremely atrophic edentulous jaws such as the vertical and horizontal insufficiency of bone and the inferior alveolar canal, the all - on - four concept has led to rehabilitate the completely edentulous jaw with minimum bone volume and avoiding bone augmentation procedures. It is the placement of four implants with two of the posterior tilted implant in the anterior portion of the edentulous jaw to allow the application of fixed prostheses with up to 12 teeth immediately.
In recent years, it has been reported that computer - guided flapless all - on - 4 treatment is a predictable procedure with a high success rate. Until recently, laboratory scanners were used for the digitalization process in edentulous jaws. The information needed for a CAD / CAM restoration in edentulous jaws was acquired extraorally based on an impression or a model cast. This presentation describes a technique that implements digital workflow and enables the clinician to place dental implants, and fabricate an immediately loaded provisional prosthesis as well as a definitive fixed implant prosthesis without conventional impressions or models. Especially in extremely atrophic edentulous cases, there is a lack of keratinized attached gingiva. For this reason, many clinicians consider flap implant surgery in those cases. All-on-4 treatment concept has been a very predictable treatment protocol for fully-edentulous patients. With sophisticated surgical techniques, placing four implants per arch can be a standard procedure with long term high survival rate.
However, from many literatures, we realized that mechanical complications are major problems that many clinicians are facing. By proper analyzing force distribution and carful material selection, meticulous occlusion design, it will be possible for clinicians to reduce the incidence of mechanical complications.
In this lecture, a newly developed All-on-4 denture design incorporating BioHpp and titanium will also be addressed, along with the mid-term follow up results of this application. -

" Clinical Experience and Experimental Animal Research With Bone Grafts and Dental Implants for Long - Term Success "

Bone grafts in oral and maxillofacial surgery may be used to reconstruct areas in the jaws which have bone lost as a result of trauma, tumour surgery or congenital defects. Bone loss are also common for people who have lost their teeth or had gum diseases. Sites that lack the necessary bone can be improved with the use of bone graft for dental implants.
Clinically, having good sound jaw bones and improved oral health care are important in the placement of dental implants for oral rehabilitation of patients. Bone grafts are needed for resorbed bone, bony defects and implants in strategic sites for functional and aesthetic success. Our experimental animal study on bone graft materials indicated more bone formation when bone ceramic was used with piezosurgery or trephine harvested autograft as compared to bone ceramic alone.
There are many clinical challenges using bone grafts and dental implants for the replacement of missing teeth by the dental practitioners to get predictable and successful results. Today, most patients want implants to feel like real teeth and to eat properly after oral rehabilitation with dental implants besides improving their quality of life. This presentation is to show successful cases by sharing clinical as well as local research experiences with bone grafts and dental implants. -

" Dense Polytetrafluoroethylene ( d-PTFE ) Membrane in Socket Preservation and Ridge Augmentation Using the Open Membrane Technique Socket Preservation "

Resorbable membranes and e - PTFE membranes were developed and widely used for guided bone regeneration, but complete flap coverage was needed to prevent early wound exposure. To avoid the complications of membrane exposure, extending a tension-free flap to close the wound is essential when using a resorbable membrane or an e-PTFE membrane. However, the advancement of the flap will decrease the width of the keratinized mucosa, reduce the depth of the oral vestibule of the edentulous area, and cause coronal movement of the mucogingival junction.
In contrast to resorbable membranes and e - PTFE membranes, the d - PTFE membrane has been designed to provide some advantages: low porosity, high density to prevent migration of bacteria into the membrane and quick surgical procedure without primary closure.
The open membrane technique using d - PTFE membranes can preserve the volume of sockets following tooth extraction. The use of d-PTFE membranes without primary closure also allows clinicians to preserve the existing keratinized mucosa width and to achieve the regeneration of keratinized mucosa over the extraction socket and grafted implant site.
This lecture will review the currently available literature on the outcome of using d - PTFE membranes in socket preservation and ridge augmentation with the open membrane technique and share the clinical experiences of Dr. Cheng on this technique. -

傳統做法不外乎是由下顎棚或是下顎聯合區取骨塊植在臼齒區,以增加植牙區骨高度,方能進行植牙。但是這兩種方式不但造成兩處傷口,造成患者不適,且長期觀察其成功率並不高。
利用下顎臼齒區下顎神經管偏向舌側的解剖學特性,以舌側斜向頰側的方式,將植體斜向植入頰側區,再以個性化基台完成修復,是完美處理好下顎臼齒區骨高度不足的新選擇。

The overall color of the restoration is the most important to secure the esthetic. The esthetic in the place where the tooth erupts that is the cervix is very important. The securing of esthetic becomes difficult in the state that a marginal exposure due to gingival recession. The restoration is in a condition the eruption with the natural feeling by so - called the emergence profile. We achieved a good result by changing profile in the area of cervix. This method named The S shape profile prevents the marginal recession for a long term while being very simple and easy and can secure the esthetic result. I want to report it mainly on the case that I treated by this concept by this lecture.
